Bring the health concern in your own words.
Write down what you want help with, what you have tried, and what a useful change would look like. List your current medicines and supplements, and bring any records you would like to discuss.
Mark the question you most want answered. Ask for an explanation when a term is new to you.
Ask about the person.
- Who will check my health, and what is their full name?
- Which licence number covers your care in the state I will occupy when we meet?
- If you rely on a state exception, which rule is it and why does it cover me?
- Which pharmacy will fill a prescription?
- Which state boards can confirm your licence and the pharmacy’s licence?
Ask about the visit.
- What do you need to learn about my health?
- Can I ask new questions after the first form?
- What examination or tests might be needed, and why?
- Who discusses the results with me?
- What happens if a medicine is not the right fit?
Ask about the medicine.
- What exact medicine is being proposed?
- What health problem is it meant to help?
- Which study supports this medicine for that health problem?
- If a pharmacy will make it for me, why can’t an approved medicine meet my need?
- Can you show me the approval letter and official medicine label for any claimed FDA approval (US Food and Drug Administration)?
- If it is pharmacy-made, can the pharmacist show me the approved ingredient, drug-quality standard, or FDA ingredient-list entry it relies on?
- If it copies an available medicine, what change makes a medical difference for me?
- What is still uncertain?
Ask about the full bill.
- What is included in the total?
- Are the medicine, visits, tests, and ongoing fees separate?
- What changes after an offer ends?
- What happens if coverage is declined or no prescription is issued?
- What are the payment commitment and cancellation terms?
Ask about follow-up.
- How can I reach the person treating me or the pharmacist with a new question?
- What written information comes with the medicine?
- What signs call for prompt or urgent help?
- What do I do if the label or delivery is unclear?
- When will the plan be reviewed?
Leave with the answer, or a named next step.
Ask who will find any missing answer and how it will reach you. Keep the product name and written quote with your notes.
Patients should have a way to raise follow-up questions after their visit, according to guidance from state medical boards. [a]
